How to Change Your Support Coordinator for DDD

If you or a loved one is receiving services through New Jersey’s Division of Developmental Disabilities (DDD) and you find that your current Support Coordinator is not meeting your needs, it’s important to know that you have the right to change to a different agency. Here’s a guide to help you navigate the process smoothly.


Step 1: Research Your Options


Begin by researching other agencies that provide Support Coordination in your county. You can find a list of Medicaid/DDD-approved Support Coordination Agencies and other relevant information at :



Step 2: Evaluate Potential Agencies


Think about the following:

  • Does the agency understand your needs?

  • Are they familiar with your community and available supports?

  • Do you feel comfortable working with them?


Step 3: Make Your Decision


After evaluating your options, identify your first-choice agency and a second-choice agency (in case your first choice is not available or does not serve your county). If you prefer, you can also opt for DDD to auto-assign a new agency for you.


Step 4: Complete the Change Form


To initiate the change, complete the Support Coordination Agency Change Form. You can send the completed form to DDD via email at DDD.SCAChoice@dhs.nj.gov. If you’re unable to submit the form by email, you can mail it to:

Step 5: Follow Up


After submitting your request, it’s a good idea to follow up with DDD to ensure that your request is being processed. Changes are typically made at the beginning of the month.
