What does DDD stand for?DDD stands Division of Developmental Disabilities.

What services does NJ DDD provide?Available services include assistive technology, behavioural supports, day programming, employment options, therapies, transportation, self-direct both employees and support services, classes in the community.

Who qualifies for DDD services as an adult in NJ?NJ resident, Be Medicaid eligible, individuals must prove that they have a chronic physical or intellectual disability that was documented prior to turning 22 years of age. Such as self-care, learning, mobility, communication, self-direction, economic self-sufficiency, the ability to live independently.

What does it mean to be Medicaid eligible?Medicaid eligible means that an individual has applied for and been enrolled in Medicaid and meets the income and financial criteria for Medicaid. Every NJ resident who qualifies for and receives federal SSI automatically receives NJ Medicaid; this should be completed when the individual turns 18 years of age.

What is the process for DDD eligible clients?DDD eligible clients must complete the DDD Application for Eligibility, and go through intake process, should include all documents from under the age of 22 years of age.

What is the purpose of an NJCAT assessment?After the preliminary review DDD will schedule a NJCAT assessment. (NJ Comprehensive Assessment Tool) This test determines the individual's money budget which is allotted for services. These 3 areas are self-care, behavioral and medical.

What is the Supports Program?The Supports Program is a Medicaid program that provides services for eligible adults with intellectual and developmental disabilities age 21 and older, living with their families or unlicensed settings. To enroll in and access services through the supports Program, individual first must be determined DDD eligible and Medicaid eligible.

How is a Support Coordination Agency chosen?An individual /family choose a Support Coordination Agency or automatically assigned from DDD.

What are the responsibilities of a Support Coordinator?Support Coordinator will be your primary contact for services, SC will connect the individual/family with resources in the community. SC agency is available for 24/7 emergency situations, and can schedule other interactions with the individual /family at their convenience. The Support Coordinator will foster a good relationship with the family and develop an understanding of the individual's needs. Be knowledgeable of services and other resources in the community. Explain Supports Program vs Community Care Program. Help individuals reach their goals dreams and identify their hopes. Plan development of ISP and other services that the individual needs, including but not limited to coordination of services, arranging therapies, Day Habilitation Services, Transportation.

What are the three steps of the Service Planning Process?The three steps of the Service Planning Process: The individual works with the service planning team to develop a plan: Individual, SC, parents, any service provider or additional person approved by the individual whose participation is necessary to develop a complete effective plan. Support Coordinator completes the Person-Centered Planning Tool (PCPT) this information is about the individual's hopes, dreams, and goals. The PCPT is written by the SC in collaboration with the individual and the family and other identified members of the team. The PCPT is completed before the individualized service plan is completed. (ISP) The SC develops the Individualized Service Plan (ISP); this document directs and prior authorizes all DDD services and service providers. This means the individualized budget will only pay for services that are prior authorized in approved ISP Plan. The ISP identifies their outcomes (goals) and together with the SC and planning team members choose appropriate services to reach outcomes. The individual services, service providers and service-related outcomes are documented in the ISP. The ISP must be developed and approved within 30 days of program enrollment and renewed annually. The ISP can be changed if an individual's needs change or goals. Any changes in services in service providers must be documented and prior authorized through the ISP.
